Contraceptive Skin Patch

The contraceptive skin patch is a hormonal birth control method that delivers estrogen and progestin through the skin via a thin, adhesive patch applied to the body. The patch is approximately 2 inches square and is designed to be worn continuously for one week, then replaced with a new patch for a total of three weeks, followed by one patch-free week during which menstruation typically occurs. This method offers the convenience of weekly rather than daily dosing while providing contraceptive effectiveness similar to oral contraceptive pills.

The hormones in the contraceptive patch work through the same mechanisms as combination oral contraceptives: suppressing ovulation, thickening cervical mucus to impede sperm penetration, and thinning the endometrial lining. The patch delivers hormones transdermally (through the skin) directly into the bloodstream, bypassing the gastrointestinal tract. This can be advantageous for women who experience nausea with oral contraceptives or have conditions that affect absorption of oral medications.

The patch can be applied to clean, dry skin on the buttocks, abdomen, upper outer arm, or upper torso (excluding the breasts). The application site should be rotated weekly to prevent skin irritation. The patch is designed to stay in place during normal activities including showering, swimming, and exercising. If the patch partially or completely detaches, specific guidelines exist for replacement based on how long it has been off and when in the cycle it occurs.

Benefits of the contraceptive patch include convenience, consistent hormone delivery, and the same non-contraceptive benefits as oral contraceptives such as cycle regulation and reduced menstrual symptoms. However, the patch may be associated with slightly higher estrogen exposure compared to oral contraceptives, which could theoretically increase the risk of blood clots. Side effects are similar to those of oral contraceptives and may include application site reactions, breast tenderness, nausea, and mood changes. The patch may be less effective in women weighing more than 198 pounds.
